Te Llamas - Informally, I have learnt in theory, that it should be correct although Que te llamas is formal!

As Prahlad said, the question is always formulated as "¿Cómo te llamas [tú]?", or if you want the formal version of it then you need "¿Cómo se llama [usted]?"
You can drop the pronouns tú and usted, they are not necessary.

Never say "¿Qué te llamas?" - it must always be "¿Cómo te llamas?"
"¿Cómo te llamas?" is the form most commonly used.
