1. None of the examples use 'usted' or 'ustedes'. Can you use those or is gustar just not used in a formal way?
2. What if I want to say something like "Sarah likes David"? Do I still have to include the indirect pronoun? And use "a xxx" for the name of the person doing the liking?
like: "David le gusta a Sarah"

1. yes, you can also use this structure with usted and ustedes - here are two examples I just added to reflect this:
Ustedes les gustan a ellos.
They like you [formal plural]
Usted le gusta a ella.
She likes you [formal]
2. If you are using specific names then you need the more conventional structure using a + name, so:
A David le gusta Sarah.
David likes Sarah.
A Javier le gustan Andrea y Carmen.
Javier likes Andrea and Carmen.
