The correct expression is ¿Qué significa...? (What does ... mean?).
For example:
¿Qué significa esta palabra? = What does this word mean?
We use significa because it's the verb significar (to mean). We don't need es la in this structure.
You could also say ¿Cuál es el significado de esta palabra? (What is the meaning of this word?), but that's a different construction.
