Is there any guidance at all as to which of the accepted placement options is preferred in a given situation? Is the choice totally down to the speaker? Which option is/are most commonly used?

I'd say it's up to the speaker and what you want to emphasise in the sentence. For me, particulary it comes more natural putting it in the beginning and if there is an explicit subject, always after the subject:
Marta ya ha llegado.
