Is "cómo está usted" better, worse, or equivalent to "cómo está" with respect to a formal response?

You can use the pronoun usted or drop it if you want - it doesn't make any difference. They are equivalent. We tend to drop the pronouns unless they are necessary to emphasize "who" or for clarification.
