Converting adjective to adverb

I took the feminine form of the adverb and added mente and it's wrong.  Why?

I suppose you're referring to the example "Siéntate cómodamente". In this case, we have the adjective "cómodo", whose feminine form is "cómoda" and not "cómode" which is what you had entered as your response in the quiz. So, to the feminine form "cómoda", we add the ending -mente, and there we have the adverb "cómodamente".
